The DHHS Community Care Program, part of the Office of Population Health Equity in Ellsworth, Maine, is dedicated to making vital health programs and social services accessible to all Mainers, especially those facing challenges in connecting with the resources they need. Through our online referral system, we provide individuals and families with streamlined access to a variety of essential services, including culturally and linguistically appropriate support, maternal and child health resources, healthcare navigation assistance, and health education. We also offer options for food, heating, transportation resources, stress management, and COVID-related assistance. With just one referral per household, our compassionate team will review the submission and connect individuals with the right resources, typically within 1-3 days. Each service has its own eligibility criteria and may vary by region, ensuring tailored support for everyone in our community.
