The DHHS Community Care Program, part of the Office of Population Health Equity in Durham, ME, is dedicated to removing barriers that Mainers face in accessing essential health programs and social services. Our user-friendly online referral system is designed for everyone, particularly those who may experience difficulties obtaining the resources they need. By filling out our referral form, individuals can connect with a diverse array of services, including culturally and linguistically appropriate support, resources for mothers and children, navigation assistance for healthcare, health and wellness education, and food resources. We also offer help with stress, anxiety, and COVID-related issues. After submitting a referral—limited to one per household—our team aims to engage with the requester within 1-3 days, providing vital connections to organizations tailored to their specific needs.
