The DHHS Community Care Program, part of the Office of Population Health Equity in Caribou, ME, is dedicated to ensuring that all Mainers can access vital health programs and social services, especially those who face barriers to obtaining the help they need. Through our user-friendly online referral system, we connect individuals with culturally and linguistically appropriate resources, including support for mothers and children, healthcare navigation assistance, health and wellness education, and options for food, heating, and transportation. Additionally, we provide resources for managing stress and anxiety and offer support related to COVID-19. By completing a single referral form per household, community members will be matched with organizations prepared to discuss their specific needs and potential options within just a few days. Each program has distinct eligibility criteria, and availability may vary by region, ensuring that our services meet the diverse needs of the Caribou community.
