The DHHS Community Care Program, part of the Office of Population Health Equity in Brewer, ME, is dedicated to empowering Mainers by providing seamless access to vital health programs, social services, and a wealth of community resources. Designed with inclusivity in mind, our online referral system helps individuals, especially those facing barriers, connect efficiently to services that meet their unique needs. Through our referral form, individuals can access culturally and linguistically appropriate support, services for mothers and children, healthcare navigation assistance, health education, and options for food, heating, and transportation. We also provide resources to manage stress, anxiety, and COVID-related challenges. We believe in responsive support; generally, our partners will reach out within 1-3 days to discuss needs and available options, ensuring that every person receives the attention they deserve.
