The DHHS Community Care Program, part of the Office of Population Health Equity in Augusta, ME, is dedicated to helping individuals and families access essential health programs and social services across the state. Through our streamlined online referral system, anyone can easily connect to a diverse array of services tailored to their needs, including culturally and linguistically appropriate support, resources for mothers and children, assistance with navigating the healthcare system, education on health and wellness, and options for food, heating, and transportation support. Additionally, we provide resources for managing stress and anxiety, as well as COVID-related assistance.
