Located in Chestertown, MD, Cancer Transitions is dedicated to supporting individuals who have survived cancer treatment, offering an evidence-based program tailored specifically for those who have completed their therapy within the last 24 months. However, survivors beyond this timeframe are also welcomed to benefit from our comprehensive services. Our program focuses on empowering cancer survivors through nutritional support, personalized exercise plans, emotional health education, and insightful medical management guidance. We strive to ensure that each participant feels cared for and equipped for a healthier future during their journey of survivorship.
