Living with Type 2 Diabetes Program

Located in East Falmouth, MA, the Living With Type 2 Diabetes Program is designed to empower individuals newly diagnosed with type 2 diabetes through free, accessible education and support over a comprehensive 12-month period. Participants receive six informative e-booklets tailored to their stage of diagnosis and enjoy a monthly e-newsletter featuring seasonal tips, resources, and information about local community events, all offered in both English and Spanish. By providing a welcoming environment, the program also invites enrollees to participate in a monthly Q&A series with ADA experts, allowing them to engage with diabetes professionals, ask essential questions, and learn about the critical connection between diabetes and heart health. Through these resources, we aim to help each participant thrive as they navigate their journey with diabetes.
