Cancer Transitions, located in East Falmouth, MA, is dedicated to empowering cancer survivors through a comprehensive, evidence-based program tailored for those who have recently completed treatment. While the program primarily targets individuals who have finished their treatment within the last 24 months, it is designed to support survivors at any stage of their journey. Participants receive valuable resources including personalized nutritional guidance, customized exercise plans, and essential education on emotional and medical health management.
