Located in Neodesha, KS, Cancer Transitions is a compassionate and evidence-based program dedicated to supporting cancer survivors who have completed their treatment, specifically within the last 24 months but also welcoming individuals at any stage of their survivorship. Our mission is to empower survivors with essential resources including nutritional support to promote wellness, tailored exercise plans to enhance physical strength, emotional health education for overall well-being, and medical management guidance to navigate post-treatment challenges. We strive to create a nurturing environment where cancer survivors can thrive and find community connection.
