Located in Lyons, Kansas, Cancer Transitions is a dedicated program designed to support cancer survivors who have completed their treatment, particularly those within the last 24 months. Our mission is to empower individuals on their survivorship journey by offering vital resources that include nutritional support, customized exercise plans, emotional health education, and guidance in medical management. We believe that survivorship can be a time of renewal and healing, and our evidence-based approach ensures that all participants receive the personalized care they need to thrive beyond their diagnosis.
