Located in Hays, KS, Cancer Transitions is dedicated to supporting cancer survivors who have completed treatment, particularly those within the last 24 months, although our services extend to anyone at any stage of their survivorship journey. Our evidence-based program focuses on equipping survivors with the essential tools for a healthier life through comprehensive nutritional support, personalized exercise plans, emotional health education, and guidance on medical management. We believe that every survivor deserves the resources and care to thrive beyond their diagnosis, fostering a community of hope and resilience.
