Cancer Transitions

Cancer Transitions is a compassionate program located in Fredonia, KS, dedicated to supporting cancer survivors who have completed their treatment. Tailored for individuals within 24 months post-treatment, the program also welcomes those who are further along in their survivorship journey. We focus on empowering survivors through essential services such as nutritional support, personalized exercise plans, emotional health education, and comprehensive medical management education. Our mission is to provide survivors with the tools and resources they need to thrive in their new normal, fostering a sense of community and resilience among those navigating life after cancer.
