Living with Type 2 Diabetes Program

The Living with Type 2 Diabetes Program in West Lafayette, IN, is dedicated to supporting individuals recently diagnosed with type 2 diabetes through comprehensive, free education and resources over a 12-month period. Participants have access to six informative e-booklets tailored to their needs after diagnosis, as well as a monthly e-newsletter packed with seasonal tips and local community event information in both English and Spanish. Enrollees who provide their phone number can join our monthly Ask the Experts Q&A series, allowing them to engage directly with American Diabetes Association experts, ask questions about diabetes management, understand the connection between diabetes and heart health, and enjoy various giveaways aimed at enhancing their journey.
