Living with Type 2 Diabetes Program

The Living with Type 2 Diabetes Program, located in Frankfort, IN, offers a supportive and educational environment for those newly diagnosed with type 2 diabetes. This free program, available in both English and Spanish, spans 12 months and is designed to empower participants with essential knowledge and effective diabetes management strategies. Through six comprehensive e-booklets tailored to meet individuals at their specific stages of diagnosis, along with a monthly e-newsletter filled with seasonal tips and community resources, participants can access valuable information at their own pace. Enrolled individuals are also invited to join our monthly 'Ask the Experts' Q&A sessions, allowing them to connect directly with Diabetes Association experts to address their questions, understand the important connection between diabetes and heart health, and gain valuable resources to help them manage their condition. Together, we strive to create a nurturing community that fosters health and well-being for everyone affected by diabetes.
