Located in Columbia City, IN, Cancer Transitions is a compassionate program dedicated to supporting cancer survivors who have recently completed treatment. While our primary focus is on individuals who have finished their treatment within the last 24 months, we welcome survivors at any stage of their journey. Our evidence-based initiative offers comprehensive support tailored to the unique challenges faced by survivors, including nutritional guidance, personalized exercise plans, emotional health education, and insights into medical management.
