Located in Joliet, IL, Cancer Transitions is a compassionate program designed for cancer survivors who have successfully completed treatment. Aimed particularly at those who finished their treatment within the last 24 months, our services extend support to all survivors at any stage of their journey. We focus on empowering individuals with essential resources, including nutritional support to foster a healthy diet, personalized exercise plans to enhance physical well-being, emotional health education for mental resilience, and guidance on medical management to navigate life post-treatment. Our mission is to provide a holistic approach that nurtures the body and spirit, helping survivors thrive as they transition into this new chapter of their lives.
