Located in Seminole, FL, Nutrition for Longevity (N4L) is dedicated to providing essential Home Delivered Meals and Medically Tailored Meals to individuals eligible under Medicare Advantage, Medicaid Long-Term Services and Supports, and the Older Americans Act. Our program focuses on delivering nutritionally balanced meals, crafted by a Registered Dietitian, to support health and wellness for various medical conditions such as Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure. We prioritize dietary restrictions, ensuring our meals are free from dairy, peanuts, gluten, and pork. For those eligible through Medicaid and the Older Americans Act, N4L works closely with clients to determine meal delivery benefits through their Case Managers or local Area Agencies on Aging. Additionally, Medicare Advantage beneficiaries may access meal services as part of their chronic care, while self-pay individuals can enjoy personalized meal options at a cost-effective price of $7.75 per meal. Beyond meal delivery, Nutrition for Longevity also provides Medical Nutrition Therapy and Nutrition Counseling, ensuring comprehensive support for our clients’ nutritional needs.
