Home Delivered Meals, based in Port Charlotte, FL, provides essential nutrition through our Home Delivered Meal and Medically Tailored Meals programs for clients eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports, and the Older Americans Act. Our meals, thoughtfully planned by Registered Dietitians, cater to various health needs—including Chronic Kidney Disease and Congestive Heart Failure—while ensuring our offerings are free from dairy, peanuts, gluten, and pork. We empower clients through our self-pay program, allowing anyone the convenience of personalized meal delivery without insurance constraints for just $7.75 per meal. Eligibility for services varies by program, and potential clients are encouraged to consult their Case Manager or local Area Agency on Aging for assistance. With additional offerings such as Medical Nutrition Therapy and Nutrition Counseling, we are dedicated to supporting the well-being and health of our community.
