Home Delivered Meals

Based in Lake City, FL, Nutrition for Longevity (N4L) is dedicated to enhancing health and wellness through our Home Delivered Meals program, which delivers nutritious, medically tailored meals to clients eligible for Medicare Advantage, Medicaid Long-Term Services and Supports (LTSS), or the Older Americans Act. Our meals are expertly planned by Registered Dietitians to cater to specific dietary needs, covering 36 medical conditions such as Chronic Kidney Disease and Congestive Heart Failure, while ensuring they are free from common allergens like dairy, peanuts, gluten, and pork. For those under state or program eligibility, our meals may be covered by Medicaid or the Older Americans Act; if unsure, individuals can check with their Case Manager or local Area Agency on Aging for assistance. Additionally, we provide a self-pay option for individuals seeking the convenience of home-delivered meals at an affordable rate of $7.75 per meal, allowing freedom in meal selection. Beyond meal delivery, N4L offers Medical Nutrition Therapy and Nutrition Counseling to further support our clients' health journeys.
