Located in New Fairfield, CT, Cancer Transitions is a dedicated program designed specifically for cancer survivors who have completed treatment, offering vital resources in their recovery journey. With a focus on individuals who have finished treatment within the last 24 months, our evidence-based approach also extends support to those further along in their survivorship. Participants can access essential services including nutritional guidance, personalized exercise plans, emotional health education, and medical management support.
