Cancer Transitions is a compassionate program located in Mystic, CT, dedicated to empowering survivors of any cancer diagnosis who have recently completed their treatment. While designed for individuals within 24 months of finishing their treatment, our resources and support can benefit anyone in their survivorship journey, regardless of when they completed treatment. We offer essential services that include nutritional guidance, customized exercise plans, emotional health education, and medical management assistance, all aimed at enhancing the quality of life for cancer survivors. Our mission is to provide a welcoming environment where individuals can find the support they need as they navigate their path forward.
