Located in Rifle, CO, Nutrition for Longevity (N4L) is dedicated to ensuring that eligible individuals have access to nutritious meals through our Home Delivered Meals program. We provide medically tailored meals designed by registered dietitians to support overall health and address specific medical needs, including conditions such as Chronic Kidney Disease and Congestive Heart Failure. Our offerings are specifically designed for those covered by Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS) programs, and the Older Americans Act, while also being available for self-pay clients who value convenience and quality. Each meal is crafted without dairy, peanuts, gluten, or pork, ensuring a safe dining experience for all. Eligibility for our services may vary, and clients are encouraged to consult their Case Managers or local Area Agencies on Aging for assistance. With our self-pay program priced at just $7.75 per meal, we’re committed to providing accessible and wholesome food options for everyone who needs it. Additionally, N4L offers Medical Nutrition Therapy and Nutrition Counseling to further support our clients' dietary needs.
