At Nutrition for Longevity (N4L), we are dedicated to providing essential Home Delivered Meals and Medically Tailored Meals to those in need across Peyton, Colorado. Our programs cater to clients eligible for Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), and individuals seeking self-pay options. Each meal is crafted by a Registered Dietitian to fulfill nutritional needs for overall health and is specifically designed to accommodate up to 36 medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, while ensuring they are free from dairy, peanuts, gluten, and pork. For Medicaid and Older Americans Act recipients, meal delivery eligibility varies, and we encourage consultation with your Case Manager or local Area Agency on Aging for guidance. Medicare Advantage beneficiaries can inquire if their plan includes meal delivery. For those without coverage, our self-pay program offers affordable meal choices at just $7.75 per meal, providing the convenience of tailored home-delivered nutrition. Additionally, we offer Medical Nutrition Therapy and Nutrition Counseling to further support our clients' health journeys.
