At Nutrition for Longevity (N4L) in Milliken, CO, we are dedicated to enhancing health and well-being through our Home Delivered Meals program. We offer Medically Tailored Meals specifically designed to meet the nutritional needs of clients under Medicare Advantage plans, Medicaid Long-Term Services and Supports programs, or the Older Americans Act. Our meals, crafted by a Registered Dietitian, cater to diverse medical conditions including Chronic Kidney Disease and Congestive Heart Failure, and are formulated to be free from dairy, peanuts, gluten, and pork. N4L also provides a self-pay option for those who do not qualify for government assistance, ensuring that anyone can access nutritious meals for just $7.75 each, with customizable selection and delivery. For Medicare Advantage beneficiaries, we participate in select PPO plans for post-discharge meal support. Additionally, we provide Medical Nutrition Therapy and Nutrition Counseling to empower our clients in their health journey.
