Home Delivered Meals

Located in Las Animas, CO, Nutrition for Longevity (N4L) is dedicated to improving health outcomes through our Home Delivered Meals program, offering nutritious, medically tailored meals specifically designed for individuals with varying health conditions including Chronic Kidney Disease and Congestive Heart Failure. Our meals, planned by a Registered Dietitian, are tailored to meet the specific nutrient requirements for overall well-being, and are delivered free from dairy, peanuts, gluten, and pork. We cater to clients who are covered by Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), or the Older Americans Act, as well as providing an affordable self-pay option at $7.75 per meal for those not qualifying for government assistance. Our commitment extends beyond meal delivery; we also offer Medical Nutrition Therapy and Nutrition Counseling to support our clients' health journeys. For those interested, eligibility for meal delivery can be determined through a Case Manager or by contacting Medicaid providers or local Area Agencies on Aging.
