Home Delivered Meals

Located in Fort Collins, CO, Nutrition for Longevity (N4L) is dedicated to enhancing health and well-being through our Home Delivered Meals and Medically Tailored Meals programs. We serve eligible clients covered under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act, as well as individuals seeking self-pay options. Our meals are thoughtfully crafted by Registered Dietitians to address the nutritional needs of clients with various medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, and are free from dairy, peanuts, gluten, and pork. We provide meals to beneficiaries under most Medicaid and Older American Act programs, with eligibility varying by state and program. For Medicare Advantage recipients, we offer meal delivery as part of select post-discharge support programs. Our self-pay plan ensures anyone in need can access healthy meals at just $7.75 per meal, with a flat rate shipping fee. In addition to meal delivery, N4L provides Medical Nutrition Therapy and Nutrition Counseling to support our clients' health goals.
