Our compassionate case managers are here to guide you through every step of the process, ensuring you have access to crucial resources such as housing assistance, nutritious food, medical transportation, and emergency financial aid for bills like utilities and phone services. We offer budgeting support, naloxone distribution, and help with obtaining vital documents, all aimed at promoting your well-being and stability. To begin your journey with us, simply provide a case manager with proof of HIV diagnosis, which can be submitted in person or via fax. Our services are offered at no cost through the Ryan White Program, reflecting our commitment to empowering individuals with the tools and support they need to thrive in their communities.
