Nutrition for Longevity (N4L) is dedicated to supporting the health and well-being of individuals in Evergreen, CO, by providing Home Delivered Meals and Medically Tailored Meals specifically designed for those eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), or through the Older Americans Act. Our meals, crafted by a Registered Dietitian, cater to the nutritional needs of clients with various medical conditions, including Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure, while being free from dairy, peanuts, gluten, and pork. N4L serves Medicaid and Older Americans Act recipients while also offering a self-pay option for those seeking nutritious home-delivered meals at an affordable rate of $7.75 per meal. We aim to make meal delivery accessible and valuable for everyone, providing not only nourishment but also Medical Nutrition Therapy and Nutrition Counseling to enhance overall health.
