Our dedicated case managers guide clients through every aspect of care, ensuring access to essential resources such as housing support, nutritious food, medical transportation, and emergency financial assistance for bills like utilities and phones. We also provide budgeting help, Naloxone distribution, and vital documents assistance to empower clients on their journey to wellness. To begin accessing these vital services, individuals must share proof of diagnosis with a case manager, which can be submitted in person or via fax. Our services are provided at no cost through the Ryan White Program, reflecting our commitment to offering practical support and nurturing hope in our community.
