Nutrition for Longevity (N4L) is dedicated to enhancing health and well-being in Delta, CO, by providing Home Delivered Meals and Medically Tailored Meals for individuals eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act. Our nutritionally balanced meals are carefully crafted by Registered Dietitians to support a variety of medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, while accommodating dietary preferences by being free from dairy, gluten, peanuts, and pork. We offer services to beneficiaries of Medicaid and the Older Americans Act, with eligibility varying by program. If you have a Case Manager, please consult them to determine your eligibility. For Medicare Advantage beneficiaries, certain plans may include meals as part of their care support; check your plan details or consult customer service for more information. N4L also welcomes self-pay individuals who seek the convenience of home delivery and nutritious meals at an affordable rate of $7.75 per meal, allowing for personalized meal selection with each order. Additionally, we provide Medical Nutrition Therapy and Nutrition Counseling to further support your health journey.
