Located in Calhan, Colorado, Nutrition for Longevity (N4L) offers essential Home Delivered Meals and Medically Tailored Meals to individuals benefiting from Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act. Our carefully curated meal programs are designed by Registered Dietitians to ensure that our clients receive the necessary nutrients to promote health and address specific medical conditions, including Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure. We provide meals that are free from dairy, peanuts, gluten, and pork, catering to a variety of dietary needs. N4L welcomes beneficiaries from most Medicaid and Older Americans Act programs, and eligibility may vary by state and program. If you don’t have a case manager, your Medicaid provider or local Area Agency on Aging can assist you in determining your eligibility. For those covered by Medicare Advantage, several plans may include meal delivery in their post-discharge or chronic care support offerings. Additionally, we offer a self-pay option for individuals wishing to enjoy the convenience of home-delivered meals without qualifying for government assistance, at an affordable rate of $7.75 per meal.
