At Nutrition for Longevity (N4L), located in Basalt, CO, we are dedicated to enhancing the health and wellness of our clients through our Home Delivered Meals program, which provides nutritious, medically tailored meals designed by Registered Dietitians. We cater to individuals eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports, and the Older Americans Act, as well as offering a self-pay option for those seeking healthy meal solutions. Our meals address a variety of dietary needs and medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, while being free from dairy, peanuts, gluten, and pork. We ensure that each meal meets the unique nutritional requirements of our clients. For those on Medicaid or the Older Americans Act, meal eligibility varies based on specific state programs, and consulting with a Case Manager can provide clarity on benefits. Medicare Advantage beneficiaries may find meal delivery included in their chronic care programs with some plans. Additionally, N4L supports self-pay individuals at an affordable rate of $7.75 per meal, facilitating convenient access to healthy meals for those who seek it. We also provide Medical Nutrition Therapy and Nutrition Counseling services to further support our client's health needs.
