HIV / Hepatitis C

At Santa Rosa Community Health, we are dedicated to providing comprehensive and compassionate services for individuals living with HIV and hepatitis C. Located in Santa Rosa, CA, our program offers confidential testing, primary care, specialty consultations, and medical case management tailored to meet the unique needs of each patient. We also provide essential support services including clinical pharmacy assistance, psychiatric assessments, nutritional therapy, and enrollment in the AIDS Drug Assistance Program (ADAP). Our health education and risk reduction counseling, along with support groups and PrEP/PEP options, empower our clients to take charge of their health. For newly diagnosed individuals, our HIV Outreach Worker is available to promptly assess your needs, ensuring care is accessible within 24-48 hours through in-office, hospital, or telephonic appointments. To accommodate everyone, we offer a sliding scale fee structure based on income and family size, and we accept Medi-Cal and Medicare. We encourage patients to call ahead to schedule appointments, as we prioritize your health and safety during this time.
