Home Delivered Meals, a vital program by Meals on Wheels by ACC, serves the Sacramento, CA area by providing nourishing meals directly to seniors who are homebound due to illness, recovery, disability, or other circumstances that prevent them from accessing food independently. We deliver hot, freshly prepared midday meals Monday through Friday, along with a weekly supply of five frozen meals for convenient reheating. Our compassionate approach begins with a case worker conducting a home visit to assess eligibility, explain our services, and share valuable information on topics such as fall prevention. To ensure ongoing support, participants are reassessed every three months. Our mission is to enhance the quality of life for our seniors through reliable, nutritious food assistance, ensuring they feel cared for and connected to our community.
