Cancer Transitions offers a comprehensive, evidence-based program designed to support cancer survivors in Oxnard, CA, who have completed their treatment within the last 24 months, although we welcome individuals at any stage of their survivorship. Our mission is to empower survivors through tailored services that include nutritional guidance, customized exercise plans, emotional health education, and medical management support. We strive to create a nurturing environment where cancer survivors can find the resources they need to thrive and take control of their health journey.
