Located in Toney, Alabama, Cancer Transitions offers a vital, evidence-based program designed to empower cancer survivors who have completed their treatment. Aimed primarily at those who finished treatment within the last 24 months, this program is also open to survivors at any point in their journey. Participants receive essential support in various areas, including nutritional guidance, customized exercise plans, emotional health education, and medical management resources.
