Located in Piedmont, AL, Cancer Transitions is dedicated to supporting cancer survivors who have completed treatment, offering a holistic program designed to enhance their well-being during the transition to survivorship. This evidence-based initiative caters primarily to those who have finished treatment within the last 24 months, while also being beneficial to all survivors at any stage of their journey. Participants receive personalized nutritional guidance, tailored exercise plans, and comprehensive emotional health education, alongside critical medical management support. Our mission is to empower survivors with the tools and knowledge necessary for a healthier, more fulfilling life after cancer.
