Hey! I completely understand how you feel when you've made so much progress yet you feel yourself seeming to go backwards. As for self-discipline, I think you should look back onto your past progress and how you've accomplished so much that you should be proud of. Wouldn't it be a waste to give up all your hard work and discipline when you still have a goal to reach? Perhaps try some self-motivation tactics like setting smaller, weekly goals for yourself instead of bigger, long-term ones in order to ease back into things. Even the small things like the effort to exercise and such counts towards your larger goal that I believe you can achieve! You can do it!
