This is hard to define as there are obviously a number of addictions that vary in use / action and result. Somewhat the same with abuse however physical abuse will generally be more visible / evidential

Physical signs of abuse could come in numerous different ones dependent on the substance that is being abused. It could be yellowing of finger tips, and a scent that is unfamiliar, are some examples of Tobacco abuse.

The physical signs of abuse of substances or addiction depend greatly on what drug you are using, but in general: dilated pupils, red eyes, teeth clenching, sometimes skin abnormalities such as scabs from skin picking, and possibly yellowing teeth from smoking substances. Sometime people have track marks on their arms if they use needles or nosebleeds if they snort substances.
