There are multiple organizations out there. For example, there is NAMI, which is kind of the UMBRELLA organization for mental-health.

Lots of support is out there, in regards to your disability. Depending on your disability, there is bound to be some organisation. For example, I have Tourette's Syndrome, there is an organisation for that, the TSA USA (Tourette's Syndrome Association- United States of America) and it is quite helpful. Talking to a close friend about the disability is as well helpful :) I'm sure you can find some articles and support on your disability with a Google, Bing, Yahoo!, or any other search engine.

Many communities have support services for people with disabilities. You might be able to look these services up in a phone book or through the internet.

Sometimes the support available varies depending on your age. For instance, if you are a high school student, your school should be able to connect you to resources that are available. For instance, if you are visually impaired, your school should have a teacher of the visually impaired, adapt any fonts, graphs Etc. to meet your needs ETC. If you are in college, you can visit your colleges office for students with Disabilits (sometimes academic support services or academic and advising services). If you are seeking employment, there should be a local Center for Independent Living that can help you with workplace accommodations and connect you to resources. Do not lose hope!

Your school or Human Resources department can help you find appropriate resources. Ask your doctor for the appropriate documentation and present it as soon as possible. Any ethical workplace or school will be glad to work with you to find out what suits you best, without making you stand out .Insurance companies can help you too!

That would depend on your disability, however there are multiple communities for people with disabilities. Some are generalized and others are more specific. A little research and networking should get you to the resources you need.
