Cancer Transitions is a supportive program located in Menomonee Falls, WI, designed for cancer survivors who have completed treatment. While primarily aimed at those who finished their treatment within the last 24 months, our welcoming environment and resources can benefit survivors at any stage of their journey. We focus on a holistic approach, offering nutritional support, personalized exercise plans, emotional health education, and comprehensive medical management guidance. Our mission is to empower every survivor with the tools and knowledge they need to navigate life after cancer, fostering a community of hope and resilience.
