Welcome Home Program

The Welcome Home Program in Seattle, WA, is dedicated to supporting individuals on their journey to recovery after a hospital stay by providing essential food assistance through tailored care packages. Within 48 hours of referral, participants receive 12 weeks of nutritious and flavorful meals delivered right to their homes, ensuring they have the sustenance needed to regain their strength. Additionally, our program includes access to sessions with a Registered Dietitian Nutritionist (RDN), who offers personalized guidance to empower individuals in making healthy choices. To enroll or refer someone in need, we encourage contacting your healthcare provider to complete the enrollment form and submit it to Lifelong.
