Our dedicated case management team is here to assist clients in navigating the healthcare and support systems across North Texas, providing resources such as medical case management, health counseling, oral healthcare, legal assistance, housing support, and substance abuse treatment. We believe in offering our services without discrimination, ensuring that all individuals, regardless of their background or financial situation, can receive the help they need. To access our services, potential clients must complete an intake session, with eligibility renewed every six months.
