We provide essential support such as home-delivered nutritious meals, personal care, homemaker services, and comprehensive case management to ensure individuals receive the assistance they deserve. Through a simple intake process, our Intake Specialists assess eligibility to connect qualified individuals with case managers who conduct in-home assessments and coordinate necessary services. With no income eligibility requirements and a sliding fee scale based on income, we strive to make our services accessible for all who need them.
