Nutrition for Longevity (N4L) proudly serves Pottsville, PA, by offering Home Delivered Meals and Medically Tailored Meals to eligible clients under Medicare Advantage plans, Medicaid Long-Term Services and Supports, and the Older Americans Act. Our program is designed to support the nutritional needs of individuals managing chronic health issues such as Chronic Kidney Disease and Congestive Heart Failure, providing meals developed by a Registered Dietitian to promote overall health and well-being. We ensure our meals are suitable for those with dietary restrictions, as they are free from dairy, peanuts, gluten, and pork. For Medicaid and Older Americans Act recipients, eligibility may vary; if you have a Case Manager, they can assist you in determining if you qualify. Many Medicare Advantage plans also offer meal delivery as part of post-discharge care. If you're not covered, N4L provides a self-pay option at just $7.75 per meal, allowing you to select meals and enjoy the convenience of delivery. Additionally, we offer Medical Nutrition Therapy and Nutrition Counseling services, with coverage details available through your insurance provider.
