Home Delivered Meals, a program by Nutrition for Longevity (N4L) in Phoenixville, PA, is dedicated to providing essential meal services tailored to meet the unique dietary needs of our clients. We offer Home Delivered Meals and Medically Tailored Meals to individuals covered by Medicare Advantage plans, Medicaid Long-Term Services and Supports, or the Older Americans Act. Our meals are designed by a Registered Dietitian, ensuring they meet critical nutrient requirements for various medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, while also being free from dairy, peanuts, gluten, and pork. We understand the importance of accessible nutrition, which is why we provide meals to eligible beneficiaries through Medicaid and the Older Americans Act, ensuring that affordable options are available for those who qualify. For individuals who do not qualify for government assistance, our self-pay program offers the convenience of choosing every meal for just $7.75 each, along with a flat shipping fee. Additionally, we provide Medical Nutrition Therapy and Nutrition Counseling, helping our clients achieve better health through informed dietary choices.
