Located in Perkasie, PA, Nutrition for Longevity (N4L) is dedicated to providing essential Home Delivered Meals and Medically Tailored Meals for eligible individuals covered under Medicare Advantage plans, Medicaid Long-Term Services and Supports, or the Older Americans Act. Our program ensures that clients receive meals expertly designed by a Registered Dietitian to align with their nutritional needs for optimal health, specifically catering to various medical conditions such as Chronic Kidney Disease and Congestive Heart Failure. Committed to dietary restrictions, our meals are crafted without dairy, peanuts, gluten, or pork. For those who qualify, we offer resources to facilitate meal delivery via Case Managers or local Area Agencies on Aging. Additionally, for Medicare Advantage beneficiaries, select plans may cover meals post-discharge or for chronic care support. Individuals without coverage can opt for our self-pay program, enjoying personalized meal selections at an affordable rate of $7.75 per meal, with a flat shipping fee. Beyond meal delivery, N4L also provides Medical Nutrition Therapy and Nutrition Counseling, extending our commitment to enhancing the health and well-being of our community.
