Fresh Food Farmacy, located in Kingston, PA, is dedicated to providing fresh, nutritious food and essential health education to individuals facing food insecurity. Our mission is to empower participants to manage their health more effectively through dietary choices and lifestyle modifications. We focus on supporting patients with HBA1C levels above 8 through a referral system from their primary care physicians, allowing us to deliver targeted clinical interventions. Enrolled participants benefit from diabetes education, consultations with dietitians and pharmacists, and weekly deliveries of wholesome food sufficient for preparing ten healthy meals for their families. While we prioritize community health and safety by adhering to COVID-19 guidelines, we remain committed to serving our community through food assistance and education.
