Nutrition for Longevity (N4L) is dedicated to providing essential Home Delivered Meals and Medically Tailored Meals to residents of Erie, PA, who are eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports, or the Older Americans Act. Our program ensures that clients receive meals meticulously planned by a Registered Dietitian, addressing specific nutrient needs for overall health as well as various medical conditions, including Chronic Kidney Disease and Congestive Heart Failure. We prioritize dietary restrictions, preparing meals free from dairy, peanuts, gluten, and pork. For Medicaid and Older Americans Act recipients, eligibility for our home delivery services may vary, so we encourage individuals to consult their Case Manager or local Area Agency on Aging for assistance. For Medicare Advantage beneficiaries, meals may be available through select plans as part of chronic care support, and we currently serve PPO plans. Additionally, we offer an accessible self-pay option for those seeking the convenience of home-delivered meals without insurance coverage, priced at an affordable $7.75 per meal. Alongside meal delivery, N4L provides Medical Nutrition Therapy and Nutrition Counseling to enhance overall well-being for our clients.
