Located in East Stroudsburg, PA, Nutrition for Longevity (N4L) specializes in providing Home Delivered Meals and Medically Tailored Meals to individuals covered by Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS) programs, or the Older Americans Act. Our meals, designed by Registered Dietitians, cater to a variety of health needs, including those with Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure, ensuring they meet specific nutrient requirements for optimal health. We consciously prepare meals free from dairy, peanuts, gluten, and pork to accommodate various dietary restrictions. For those eligible, including Medicaid and Older Americans Act recipients, meal delivery options may be available through local agencies or Case Managers. Medicare Advantage beneficiaries can check if their plan covers meals following a hospital discharge or in chronic care management. Additionally, our self-pay program allows individuals not covered by these plans to access our services for just $7.75 per meal, allowing full control over meal selection and delivery. N4L is committed to health and well-being through nutrition, also offering Medical Nutrition Therapy and Nutrition Counseling as part of our dedicated services.
